Hospital & Rehabilitation Referrals
When a client is preparing to return home, our team can help coordinate non-medical support for the transition.
Depending on the client’s needs and arrangements, we may provide:
- Same-day assessments
- Hourly, overnight, or 24-hour care
- Personal care assistance
- Mobility and safety supervision
- Fall-prevention support
- Meal preparation and light housekeeping
- Medication reminders
- Appointment escorts
- Support while home health, hospice, PT, OT, or speech therapy is involved
VA & Veteran Referrals
We also welcome referrals involving veterans who may need non-medical assistance at home.
Our team can help coordinate with the appropriate referral source and assist families in navigating the process for VA-authorized Homemaker/Home Health Aide services when applicable.
VA eligibility, authorization, services, and care hours are determined by the applicable VA program based on the veteran’s eligibility and care needs.
Community Referral Partners
We receive and coordinate appropriate referrals through Watershed Health and other community referral sources.
